Carousel Banner Module in Joomla 2.5

I have several issues at this URL:
www.karaokeacrossamerica.com/kaa-stuff/friends-of-kaa
If I use the default layout the slides never animate. They will scroll if I click the arrow, but never work on their own.
The module size settings do nothing in Flexisel mode.
If I use the Flexisel style the banners move, but I can't control the image size at all. They appear in the raw size of the image file even if I limit the size in the Joomla banner component itself.
I get the "Notice: Undefined variable: img_width in ......../modules/mod_carousel_banner/mod_carousel_banner.php on line 49" if I use Flexisel mode at all. If I use default mode I get no error.
There may be more once I can get some basic functionality.

Hi form your demo site, it looks like jQuery loads more than once. Please try to disable the jquery load from the carousel banner backend

I actually did that before I submitted this. I also tried loading jQuery from Google instead. Neither options resolved this.
I also tried the "patched" version that I found in another thread as an attempt to resolve the "mod_carousel_banner.php on line 49" error. It installed fine even though it said J3X, but I noticed I lost the option to even choose what jQuery options existed. I re-installed the original version.
Can you tell me what extension is loading the conflicting library? I checked to see what else may be doing this but didn't see any other optional modules/components loading. I admit I am not a serious coder though so I may miss it.
